- permission-presets: register the permissions unit synchronously before the
existing-session sweep, so a remount reads folded knob state instead of
treating every session as fresh; add a regression test for that path
- sandbox-policy/terminal-bash: mount the projection registry in the 5 pwsh
terminal tests, the sdk-minimal bundle, and the e2b fixture composition;
declare the new package dependency in both manifests
- plan-mode: restore .strict() on the plan unit state schema and drop the
deleted foldPlanMode from the bilingual READMEs

A claimed slash command consumed only the text half of the composer
submission: /goal with reference images executed, cleared the draft, and
silently stranded the images in the rail. Model-visible attachment intent
had no route through the command plane.
The submission envelope is now modeled end to end. CommandDefinition
input.images declares acceptance; the declaration rides the descriptor to
every client, onto the minted CommandClaim, and into the input machine's
claim snapshot. commands.execute carries the submission's base64 images
and enforces the declaration in the executor: non-declaring commands, a
missing attachment store, and exceeded batch limits settle as logged
error results before the handler runs. Admission reuses the attachment
package's new admitEncodedImages, extracted from api-proxy's prompt path
so both wire endpoints share one limits/validation/commit sequence.
Producers own model visibility: /goal submits one user followup (image
blocks + a fixed reference line) after a successful create/edit so goal
rounds read the images from session history; /plan folds them into its
steered message. Grammar misfits (/goal pause, bare /plan, /plan off)
return direct errors and the composer keeps the images.
On the client, enter adjudication carries a SubmitEnvelope and every
command route that cannot consume images throws a localized refusal that
renders as one composer notice with draft and images retained; the
claimed pre-gate applies the same copy. An accepting claim serializes the
draft images, forwards them to commands.execute, and clears plus releases
them only on a success outcome.
The assembled web test roster gains the ui-input-trigger and ui-commands
plugins, mirroring the shipped composition, so slash submissions exercise
the command plane; a new keyless snapshot pins the refusal banner and the
accepting /goal flow over the built client graph.
Move the triplet to implemented/architecture/, rewrite Proposal into a
present-tense Decision, fold Acceptance criteria and Risks into
Consequences (recording what pins the result), retitle the migration
section to what the move actually touched, and repoint the seven
inbound FIXME/README links.
A question may now carry `intent`, a tagged declaration that it IS a decision
of a known shape, so a UI that recognises the tag can present it as such
instead of as a generic option list. The one member is
`{ kind: 'plan-review', approve }`, which plan-mode sets on the exit_plan_mode
review.
An intent shapes presentation only: a UI honouring it answers with the same
option labels a generic UI would send, so the tool reads one answer shape
either way, and a UI that does not know the tag renders the generic flow.
`approve` names the affirmative option rather than relying on option order;
since no type can tie that label to the question's own option list, `ask()`
rejects a mismatch as BAD_INTENT, and the wire schema rejects an unknown tag
outright rather than silently rendering generic.
plan-mode also stops reporting a dismissed review as "the user cancelled
ask_user_question" — a tool it never called. A dismissal now tells the model
the user took the turn back to speak, and to stay in plan mode and wait; every
other ask failure keeps its own message.
set() on an idle agent appends plan/mode at once — no request boundary
would arrive until the next prompt, so a queued intent used to hang as
pending forever (the composer showed a dead pending target). A running
agent keeps the boundary-flush path unchanged. set() now reports which
branch ran (committed/queued/cancelled/noop); the /plan handler's copy
follows the branch (idle: "Plan mode on/off", mid-turn: the next-step
wording), and both commit paths share the header-delta narration. The
invariant drops turn enclosure: plan/mode is a standalone whole-value
event (the synthetic log-only turns removal already established the
between-turns append shape). The fixture mirrors the idle commit.
plan-mode contributes the RFC's complete double-event example: command/run
records named plan set the wanted target (off -> false, else true),
plan/mode commits and clears it, and view derives {active, pending}.
Pending is thereby a pure replay quantity recovered from the log alone.
The plan key merges into SessionProjectionMap from src/types.ts, served
through the new ./types and ./client outlets (session-title template);
compositions without the registry are unaffected.
The automation-only rewrite edited many implemented Agent Notes; several
edits replaced still-live or historical rationale instead of reframing:
- llm-model-catalog: restore the prompt/request consistency section and
selection-ownership alternatives — installAgentLlmTarget and the TUI
/model selector still ship that design; only the ACP wire is gone.
- plan-specific-collaboration-state, acp-multi-session, todo-write,
ask-user-question: link the superseding automation-only note instead
of silently rewriting the original decision or motivation; drop a
paragraph duplicating the Web-provider facts stated two paragraphs up.
- sandbox: stop claiming unit coverage for turn-enclosed config writes
(that mechanism left with the bridge) and retitle the commit-boundary
paragraph accordingly.
- Fix the missing blank line before '## Consequences' in the
plugin-command-registration pair, the JSON-RPC/Web render-intent
consumer misattribution (the second consumer is the host/client
runtime), stale bash_output/bash_kill names, and 'optional goals' in
architecture.md.
- examples/acp-agent/README.md: point at the package contract instead
of restating it; packages/ui/permission and plan-mode READMEs record
the consumer-less preset service and the exit_plan_mode coverage gap
under Known Limitations.
- 2026-06-19-acp-snapshot-tests: the new note defers the corpus
migration rather than committing to it; say so.
Re-record the touched bilingual pairs.
Address the review bot's five genuinely-new findings on the current code:
- disposal now discards any still-pending inbox items before the loop
exits, so every enqueued id gets a terminal lifecycle event.
- injection (next-step/no-wakeup) validates its payload up front, before
opening the idle one-shot turn, honoring 'invalid input throws before
any append'; and rejects attached contexts (which belong only to inbox
messages) rather than silently dropping them.
- agentMessage() freezes the agent/inbox/* payload so a listener cannot
mutate the shared correlation object mid-dispatch.
- refresh the package READMEs (compact, goal, guard, hook-protocol,
plan-mode, time-context, workspace-context) that still referenced the
removed context/message event, with the source-based user/message
distinction.
The up-front injection validation makes two finally branches unreachable
(v8-ignored as the turn-enclosure backstop). Adds regression tests for
disposal discard, context rejection, up-front validation, and the frozen
payload; per-file coverage stays 100%.
